Would you like to join our friendly and hardworking Brokerage and Support Team? Would you like to work in our adult service working with older people, adults with Learning and or physical Disabilities or those with a sensory need? We are looking for a motivated, enthusiastic individual who has good communication skills to work in our forward thinking dynamic team .
Our team consist of Social Workers, Occupational Therapists, Social Care Practitioners and many more highly skilled social care staff, all working together to support the needs of our Wokingham residents.
The team also has a sensory needs service, assistive technology service and an independent brokerage service.

We are based in the Wokingham Council offices in Shute End within short walk from Wokingham Town centre. We work closely with our Wokingham Borough Council colleagues, and are always thinking of new and innovative ways to deliver our services.

Post Description:
As a Social Care Practitioner you will ensure a support plan is facilitated and agreed to help customers use their personal budgets to achieve desired outcomes and improve their quality of life and well- being. As part of the Social Care Practitioner role, you will also then monitor and review support plan outcomes to ensure they are working well, complete an initial 6 week review and further tweak support if necessary. Where identified you will be supported to undertake relevant Assessments e.g. Carer Assessment, or a Reassessment of need.

You will carry a caseload and receive regular supervision and support as necessary. You will have a comprehensive Induction period and a “buddy.” to help you settle in.
