What’s special about this opportunity?

This is a fantastic opportunity to be involved in the development of an integrated approach to provide high quality health and social care provision for older adults. This entails working closely within a multidisciplinary team, coupled with close collaborative working with other statutory and voluntary/charitable providers.

About you
As a joint service manager you will work both strategically and operationally providing integrated social care, advice, support and therapeutic interventions to older people with mental health difficulties.

Clinically informed, you will enable the team to provide flexible treatment and support in the community with the aim of maintaining and promoting independent living for as long as appropriate and desirable utilising an approach which integrates CPA, personalisation and personal budgets. This will include interfacing with the new structures in social care for assessment, brokerage and support. Safeguarding vulnerable adults will be a high priority and there will be a requirement to chair safeguarding meetings and work with providers.

You will show great resilience, leadership and communication skills not only to support staff to engage in quality initiatives, audit and service evaluation activities to ensure high quality service provision but also to ensure there are effective systems in place to monitor, collect and report on performance and data for both Berkshire Health Foundation Trust and Optalis.

Maximising efficiently, you will take a lead in recruitment processes and be cognisant of financial constraints in service planning and delivery.

Reliable and knowledgeable, you will have great IT skills and be familiar with various systems and software packages. You will need access to your own car, and be able to travel independently to locations across Berkshire.
