We are currently recruiting Senior Social Worker within our Physical Disability and Older Persons Team. The Senior Social worker role will be part of Locality access point. The Locality Access point is a joint initiative with Health and Social Service we will provide personal and practical support to help people lead full and active lives, maintain their independence and dignity, and have choice and control as far as they can to achieve their outcomes.
The LAP will operate Monday to Friday 9am-5pm
You will come to the role with energy, experience and a good understanding of Adult social care. In return we will provide you with the tools to do your job, excellent supervision and development opportunities to progress your career.
Successful applicants will be skilled at communicating and working with people with social care needs and with people with a variety of physical and cognitive impairments. You will need to have excellent communication skills and the ability to work professionally in a multidisciplinary / multi-agency context. You will need to have competent IT skills and be able and willing to learn to use a variety of software systems.
To facilitate as required the implementation of interventions in order to meet residents identified outcomes and achieve their optimum functioning and independence using 3 conversation model (EST )
